as an emergency nurse, I find this so irritating

in the last 4 weeks I have had 4 pts who have stated they have no allergies, but their old notes or dr's letter states they do (including anaphylaxis)

this is so annoying - and then staff get the blame if something where to happen


any suggestions on better documentation. how you you deal with pts who don't remember important details relating to their health

I go with what the Dr. says. The client might not be in the mental capacity to remember that they have an allergy. Better safe then sorry. It is frustrating I agree, no idea how to improve it
