A client comes in to the emergency room complaining of persistent cough, hemoptysis, hoarseness, weight loss and fatigue. Which of the following actions, if taken by the nurse, is MOST appropriate?

a. Auscultate the lungs
b. Place client in an isolation room
c. Take vital signs
d. Administer oxygen therapy.

Correct Answer is B

Rational

The client is exhibiting sign and symptoms of tuberculosis. It is MOST important to implement Airborne Precautions and place client in respiratory isolation.

a. correct but not a priority
b. correct answer
c. correct but not a priority
d. No indication for oxygen at this time

The question was easy. It would be harder for inclex questions. like maybe have admister a PPD as a option would have made it a little more difficult
