I KNOW big step LOL....But after being a mum for 16 years i feel i need to do this for myself...Anyway i was wondering if anyone could tell where i could find some information about preparing a care plan for an elderly resident who is being admitted to a aged care facility...Ive search the web and was hoping to find an example of one to use as a reference guide....ANY HELP would be greatly appreciated......Thanks very much.....

As far as a reference guide, have you tried asking a long term facility? Often they will have standardized are plans all ready done. Good Luck...TTYL
